Borderline personality disorder is a condition of emotional intensity. Feelings arrive faster, land harder, and take longer to come down than they do for most people, and relationships, self-image, and impulse control all get pulled around by that intensity.

BPD carries more stigma than almost any diagnosis in mental health, including from clinicians who should know better. The evidence does not support the pessimism. With dialectical behavior therapy and consistent care, a large proportion of people no longer meet criteria over time. Programs available for borderline personality disorder

Golden Coast Rehab is a contracted provider of Specialty Mental Health Services (Partial Hospitalization Program and Intensive Outpatient Program) for the County of San Diego, Behavioral Health Services, under County Contract 575462. Both programs serve adults 18 and older whose primary condition is a mental health condition, and both are delivered at our La Mesa facility.

Partial Hospitalization Program (PHP)

Day treatment is appropriate during periods of crisis, frequent self-harm, or after a hospitalization, when daily structure and daily skills practice are what hold things together. PHP runs during the day, several days a week, and you go home each evening. It is the most structured level of care we offer.

Intensive Outpatient Program (IOP)

IOP provides several skills-focused sessions a week — the standard rhythm for building emotion regulation while living your ordinary life. IOP is several sessions a week rather than full days, so it fits around work, school, and family. Many people step down into IOP from PHP; others start here.

We do not provide residential or inpatient treatment. If your assessment shows you need a higher level of care than we offer, we will tell you plainly and help you find it.

What treatment looks like

Nobody walks in knowing how this works. Here is the actual sequence for borderline personality disorder treatment here, start to finish.

Intake and assessment

We look at emotional patterns, relationships, self-harm and suicidal behavior, substance use, trauma history, and what has and has not helped before. The questions about self-harm are asked directly and without judgment. Honest answers get you better treatment, not punishment. Symptoms are tracked with standardized measures, including the PHQ-9 and the GAD-7 where clinically indicated, completed at intake and repeated during treatment so that change is measured rather than assumed.

The first two weeks

Skills first, before anything else. Distress tolerance and emotion regulation give you something to do with an intensity that until now has only had bad options attached to it. Safety planning happens here too, in concrete detail.

Active treatment

DBT skills training in group plus individual therapy: mindfulness, distress tolerance, emotion regulation, and interpersonal effectiveness. Group work matters especially here, because being in a room with people who experience emotion the same way undercuts the isolation the diagnosis creates.

Stepping down and aftercare

Step-down keeps skills practice going with less structure around it. Your plan covers your triggers, your crisis steps, your supports, and the specific skills you will use — written down, because they are hardest to recall in exactly the moments you need them. Typical length of stay is around six weeks in PHP and around six weeks in IOP, but length is a clinical decision made by your treatment team based on how you are actually doing, not a fixed schedule.

Is borderline personality disorder treatable?
Yes, and better than its reputation suggests. Dialectical behavior therapy has strong evidence, and long-term follow-up studies show a large proportion of people no longer meet diagnostic criteria over time. Improvement is the expected outcome, not the exception.
What is DBT?

I have been told providers do not want to treat BPD. Is that true here?
It is a real experience many people have had, and it is not the case here. We treat this diagnosis deliberately and we staff for it.
Does medication treat BPD?
There is no medication for BPD itself. Medication can help specific symptoms or co-occurring conditions such as depression or anxiety. Therapy is the primary treatment.

How is DBT different from other therapy approaches?

Can BPD be treated alongside another diagnosis, like depression?
Yes, co-occurring conditions like depression or anxiety are common alongside BPD, and your intake assessment identifies all conditions present so your treatment plan addresses them together.
What if I've been in treatment before and it didn't help?
Prior treatment experiences, including what did or didn't work, are useful information for your intake assessment and help your care team build a more effective plan this time.
How long does DBT skills training typically take?
DBT skills training is delivered in a structured format, but the pace and length depend on your individual treatment plan and clinical progress, determined by your care team rather than a fixed timeline.
