How Do the Two Tracks Fit Together in Dual Diagnosis Treatment?

The two tracks fit together through one clinical team working under two separate authorizations: Golden Coast Rehab holds DHCS substance use disorder certification 370254AP for outpatient detoxification, outpatient, intensive outpatient, and medication-assisted treatment.

Golden Coast Rehab is a contracted provider of Specialty Mental Health Services (Partial Hospitalization Program and Intensive Outpatient Program) for the County of San Diego, Behavioral Health Services, under County Contract 575462.

One boundary matters here, and we state it plainly. The Partial Hospitalization Program enrolling today is a specialty mental health service, delivered under our County of San Diego contract. Golden Coast Rehab is licensed to provide partial hospitalization for substance use as well, but that track is not currently enrolling; today our PHP serves adults whose primary condition is a mental health condition, under County Contract 575462. Substance use care is provided at the outpatient detox, outpatient, intensive outpatient, and medication-assisted treatment levels under DHCS certification 370254AP. The two tracks are coordinated by one clinical team, but they are separately authorized and we do not blur them.

Why the Mental Health Side of Dual Diagnosis Cannot Wait

A long-standing convention in addiction treatment held that a person had to get clean before anything else could be addressed. In practice that ordering fails often, because the untreated depression, trauma, or anxiety is frequently what the substance was managing, which is why our ranked local dual diagnosis options weigh both credentials together. Take away the substance, leave the condition, and the pressure to return is enormous.

Integrated care means both are treated at once, by one team, with a single plan, and nobody is told to come back when they have some clean time before their psychiatric symptoms are addressed.

What Dual Diagnosis Treatment Looks Like

Assessment

A single evaluation covering substance use history, withdrawal risk, psychiatric symptoms, medical history, and safety. Symptoms are tracked with standardized measures, including the PHQ-9 and the GAD-7 where clinically indicated, completed at intake and repeated during treatment so that change is measured rather than assumed.

Stabilization

Outpatient withdrawal management where it is needed, under DHCS certification 370254AP, with psychiatric symptoms monitored throughout rather than after.

Active treatment

Substance use programming at the outpatient, intensive outpatient, or medication-assisted treatment level, coordinated with specialty mental health care at the PHP or IOP level under our County of San Diego contract. Medications for opioid or alcohol use disorder and psychiatric medications are managed together.

Step-down and aftercare

A relapse plan covering both conditions, since each is a reliable trigger for the other, plus continuing outpatient care and community supports.

What does dual diagnosis mean?
Dual diagnosis means a substance use disorder and a mental health condition are present at the same time. It is common rather than unusual, and it needs both conditions treated together.
Do I need clean time before you will address my depression or anxiety?
No. You don't need clean time before we address your depression or anxiety. Both are treated concurrently by one clinical team. Waiting to treat the psychiatric condition is a common reason people relapse.
Do you have a substance use PHP?
Not enrolling yet. Golden Coast Rehab is licensed to provide partial hospitalization for substance use as well, but that track is not currently enrolling; today our PHP serves adults whose primary condition is a mental health condition, under County Contract 575462. Substance use care is provided at the outpatient detox, outpatient, intensive outpatient, and medication-assisted treatment levels under DHCS certification 370254AP. Can I take medication for opioid or alcohol use disorder while on psychiatric medication?
Yes. In most cases you can take medication for opioid or alcohol use disorder while on psychiatric medication, and managing them together is the point of integrated care. Our psychiatric provider reviews the full regimen rather than treating each in isolation. See medication-assisted treatment.
Do you provide residential treatment?
No. Golden Coast Rehab does not provide residential treatment directly in either lane, though our housing partners can enable residential-type support and sometimes forms of detox alongside your outpatient care. If your assessment shows you need a higher level of residential or inpatient care than that, we will tell you and help you find it.
Does Medi-Cal cover dual diagnosis treatment?
Yes. Medi-Cal covers dual diagnosis treatment at no cost to you, on any Medi-Cal plan in San Diego County, since both lanes are administered through San Diego County Behavioral Health Services.
How do you screen for a co-occurring mental health condition?
We screen for a co-occurring mental health condition using the PHQ-9 to screen for depression severity and the GAD-7 to screen for anxiety severity as part of your intake assessment. Those results, along with your substance use history, shape a treatment plan that addresses both together.
Which mental health conditions do you treat alongside addiction?
The mental health conditions we treat alongside addiction are, formally, depression and anxiety, screened with the PHQ-9 and GAD-7. Treatment addresses whichever mental health symptoms are contributing to or complicating your substance use. Your assessment determines the specific focus of care.
Will my psychiatric medication be changed when I start treatment here?
No, your psychiatric medication won't be changed when you start treatment here without a clinical reason and a conversation with you first. Existing psychiatric medication is reviewed as part of your intake, and any changes are made deliberately, not automatically.
Is dual diagnosis treatment available by telehealth?
Yes. Dual diagnosis treatment is generally available by telehealth as part of how care is delivered here. We work out which sessions make sense in person versus by telehealth based on your treatment plan.
Does treating my mental health slow down my addiction treatment?
No. Treating your mental health does not slow down your addiction treatment, since the two run together rather than one waiting on the other. Trying to treat addiction while an untreated mental health condition keeps driving use tends to fail, which is exactly why they're addressed as one plan.
What's the difference between PHP and IOP at Golden Coast Rehab?
PHP and IOP are the two structured levels of care in the mental health lane of dual diagnosis treatment: PHP is a specialty mental health service under County Contract 575462, meeting a full day, 9am-12pm and 1pm-4pm, while IOP meets 9am-12pm Pacific, several days a week, and can serve either the mental health or substance use side of your plan. PHP is never a substance-use level of care. For the addiction side, care runs through detox, outpatient, IOP, or MAT instead.
Is dual diagnosis treatment covered as a specialty mental health service in San Diego County?
It depends on which side of your care you mean. The mental health portion of dual diagnosis treatment, including PHP, is covered as a specialty mental health service under County Contract 575462, while the substance use portion is covered under DHCS certification 370254AP through the Drug Medi-Cal Organized Delivery System. Both run through San Diego County Behavioral Health Services, coordinated as one plan rather than two separate systems you have to navigate yourself.
