Behavioral Health Glossary

Behavioral health comes with a vocabulary problem: the systems that pay for care, the programs that deliver it, and the clinicians who provide it all use terms that mean something specific — and getting them slightly wrong can send you to the wrong door. This glossary defines the terms you'll meet on this site and in the San Diego County system, in plain language. When a term has its own full page, the definition links to it.

A–C

Access and Crisis Line (ACL)

San Diego County's 24/7 behavioral health line at (888) 724-7240 — screening, referrals, and crisis support for anyone in the county, regardless of insurance. For emergencies, call 911; the 988 Suicide and Crisis Lifeline is the national equivalent. More in our 988 vs. ACL guide.

ASAM criteria

The American Society of Addiction Medicine's framework for matching a person to a level of substance use care. Used across the treatment field to structure placement decisions.

CalAIM

California Advancing and Innovating Medi-Cal — the DHCS initiative reshaping Medi-Cal, including the documentation reforms that govern how behavioral health services are billed today.

CBT (Cognitive Behavioral Therapy)

A structured therapy targeting the loop between thoughts, feelings, and behavior. First-line for anxiety and depression. See our CBT page.

Co-occurring disorders

A mental health condition and a substance use disorder present at the same time, treated together rather than sequentially. See co-occurring disorders.

Crisis Stabilization Unit (CSU)

A short-term facility for acute psychiatric crisis — distinct from an emergency room and from hospital admission. Explained in our CSU guide.

D–I

DBT (Dialectical Behavior Therapy)

A skills-based therapy built around emotion regulation, distress tolerance, and interpersonal effectiveness; the best-studied treatment for borderline personality disorder. See DBT.

Detox (withdrawal management)

Medically supervised support while the body clears a substance. Golden Coast provides outpatient detox — monitoring during the day, home at night; we do not provide residential care.

DHCS

The California Department of Health Care Services — the state agency that runs Medi-Cal and certifies substance use treatment providers. Golden Coast's SUD certification is 370254AP.

DMC-ODS

The Drug Medi-Cal Organized Delivery System — Medi-Cal's substance use benefit, administered in San Diego by the county rather than by your managed care plan. Covered in our coverage guide.

Dual diagnosis

An older term for co-occurring disorders, still widely used on the addiction-treatment side. See dual diagnosis.

EMDR

Eye Movement Desensitization and Reprocessing — a trauma therapy that pairs recalled memories with bilateral stimulation. See EMDR and what a session is like.

ERP (Exposure and Response Prevention)

The exposure-based therapy with the strongest evidence for OCD: approaching triggers deliberately while not performing the compulsion. See ERP explained.

GAD-7

A seven-question anxiety screening tool scored 0-21; one of the two instruments we administer at assessment. Full walkthrough in GAD-7 explained.

IOP (Intensive Outpatient Program)

A structured treatment schedule of several sessions per week while living at home. Golden Coast runs a mental health IOP under the specialty mental health system and a substance use IOP under DMC-ODS. See mental health IOP.

L–O

LPHA

Licensed Practitioner of the Healing Arts — the regulatory term for the licensed clinicians (therapists, psychologists, physicians) whose assessments carry diagnostic weight in Medi-Cal documentation.

MAT (Medication-Assisted Treatment)

FDA-approved medications combined with counseling for substance use disorders, most commonly for opioids and alcohol. See MAT and MAT myths vs. evidence.

Medi-Cal

California's Medicaid program — public health coverage for eligible residents. In San Diego, physical health runs through four managed care plans while behavioral health runs through county systems. Start with our Medi-Cal guide.

Managed care plan (MCP)

The plan named on a Medi-Cal card — Blue Shield Promise, Community Health Group, Kaiser, or Molina in San Diego. It handles physical health; specialty behavioral health is carved out to the county. See plans explained.

Medical necessity

The documented clinical justification a covered service requires — the assessment finding that a diagnosis (or suspected diagnosis) needs this level of care.

Optum

The county's administrative partner for San Diego behavioral health — it runs provider operations and authorizations on the county's behalf. Optum is not an insurance plan.

P–S

Partial Hospitalization Program (PHP)

The most structured outpatient level: full treatment days, home at night. At Golden Coast, PHP is a specialty mental health program under County Contract 575462. See PHP and what PHP is really like.

Peer support specialist

A certified team member with lived recovery experience who supports clients from that vantage point.

PHQ-9

A nine-question depression screening tool scored 0-27; the second of the two instruments we administer. Walkthrough in PHQ-9 explained.

Post-acute withdrawal

Lingering symptoms — sleep disruption, mood swings, cravings — that can continue after acute withdrawal resolves. See post-acute withdrawal explained.

Prior authorization

Approval from the county system before certain services begin; part of how Medi-Cal behavioral health manages care. Your provider handles most of this.

Psychiatric evaluation

A structured assessment by a psychiatric provider covering history, symptoms, and medications. See what happens in one.

SMHS (Specialty Mental Health Services)

Medi-Cal's carve-out for moderate-to-severe mental health needs, administered by counties. Golden Coast's PHP and mental health IOP operate in this system. See using Medi-Cal for mental health.

Step-down

Moving to a less intensive level of care as you stabilize — PHP to IOP, IOP to weekly outpatient. See what happens after PHP.

SUD (Substance Use Disorder)

The clinical term for addiction — a diagnosable condition with severity ranges, not a moral category. Our SUD services are detox, OP, IOP, and MAT.

T–W

Telehealth

Care delivered by video or phone; generally available as part of how outpatient care is delivered. Ask admissions what applies to your situation.

Therapeutic milieu

The treatment environment itself — the structured community of a day program, which does clinical work beyond any single session.

Trauma-informed care

An approach that assumes trauma may be part of any client's history and designs care to avoid re-traumatization. See trauma treatment.

UM (Utilization management)

The county system's review of whether services continue to meet medical necessity — the reason documentation, not a calendar, determines length of care.

Withdrawal

The body's response when a substance it has adapted to is reduced or stopped. Severity varies by substance; alcohol and benzodiazepine withdrawal can be medically dangerous — never stop abruptly without medical guidance. See tapering vs. cold turkey.
