A psychiatric evaluation looks at what is happening now, what has been tried before, and whether medication belongs in the plan at all. It is not a form you fill out once and forget. It is the starting point for a decision you make with your clinician, not one made for you.

At Golden Coast Rehab, evaluation and any medication that follows are handled by Sanjai Mathew Thankachen, MD, a board-certified psychiatrist and our Medical Director. Prescribing and monitoring happen in the same building as your therapy, so your medical and behavioral care stay coordinated rather than split across separate offices.

Why Do People Choose Medication Management Here?

People choose medication management here because the decision is not automatic. Some clients come in already convinced they need a prescription and find the conversation is more careful than that. Others arrive resistant to medication entirely and discover it is offered as an option, never a requirement, weighed against what therapy and the rest of the program can address on their own.

What the Evaluation and Ongoing Care Look Like

Evaluation at intake

The psychiatric evaluation happens as part of intake, reviewing symptoms, history, and any medications already in use. Dr. Thankachen looks at the whole picture, including how substance use and mental health symptoms may be feeding each other, before any medication conversation starts.

A shared decision

If medication is a fit, the choice of whether to start it is made with you, not for you. You hear the reasoning, the expected effect, and what monitoring looks like, and you decide alongside your clinician.

Monitoring built into the week

Psychiatric visits are scheduled inside your program week rather than as a separate errand. Dosage, side effects, and response get reviewed on a set cadence so adjustments happen quickly instead of waiting for symptoms to worsen.

Where Medication Management Fits in Our Programs

Medication management at Golden Coast Rehab is delivered inside a program rather than as a standalone service. Psychiatric visits are layered into our specialty mental health partial hospitalization and intensive outpatient programs, provided under our County of San Diego contract, running alongside the same groups and therapists across the week.

Medication for opioid or alcohol use disorder is a separate clinical track with its own prescribing protocols under DHCS certification 370254AP. See medication-assisted treatment for that program rather than this one.

Who Can Benefit From Medication Management?

Medication management can benefit adults whose symptoms are not fully addressed by therapy alone, and especially those with a mental health condition tangled with substance use. It tends to fit when these ring true:

  • Symptoms of depression, anxiety, or another mental health condition that persist despite therapy
  • A prior diagnosis with a medication history that needs review, not a blank restart
  • Substance use alongside a mental health condition, where both need to be seen by the same team. See dual diagnosis
  • Uncertainty about whether medication is even the right step, wanting that assessed rather than assumed

Why Golden Coast for Medication Management in San Diego

1. One psychiatrist overseeing the medical side

Dr. Thankachen, as Medical Director, manages medication decisions himself rather than routing you between rotating prescribers.

2. Medication decided with you

Nothing is prescribed by default. The evaluation weighs whether medication adds value alongside the rest of your program before any prescription is written.

3. Visits built into the program week

Psychiatric follow-up is scheduled inside PHP and IOP, so monitoring happens on a set rhythm instead of being left to you to arrange separately.

4. Coordinated with your therapy team

The same building holds your prescriber and your therapists, so a medication change and what surfaces in group are seen by people talking to each other, not separate offices.

5. Covered on any Medi-Cal plan in the county

Medication management delivered inside our programs is covered for San Diego County Medi-Cal beneficiaries at no cost when clinical criteria are met.

How often will I see the psychiatrist?
How often you see the psychiatrist depends on your program and where you are in treatment. Visits are scheduled inside your PHP or IOP week rather than as a separate appointment you have to arrange.
Is this the same as medication-assisted treatment for addiction?
No, this is not the same as medication-assisted treatment for addiction. Medication for opioid or alcohol use disorder runs on its own protocols under our DHCS certification. See our medication-assisted treatment page for that program.
Does Golden Coast Rehab offer medication management in San Diego?
Yes. Golden Coast Rehab offers psychiatric evaluation and medication management in San Diego County, delivered inside our PHP and IOP programs at our La Mesa facility.
Is medication management covered by Medi-Cal at Golden Coast?

What if I am already on a medication from another provider?
If you are already on a medication from another provider, the evaluation reviews it as part of your history, and Dr. Thankachen decides with you whether to continue, adjust, or change it.
Can medication management happen alongside dual diagnosis treatment?
Yes. Medication management can happen alongside dual diagnosis treatment, since the same evaluation looks at mental health symptoms and substance use together rather than as separate tracks.

About this page

Prepared by the Golden Coast Rehab team. Everything described here is delivered under two public, verifiable authorities: specialty mental health services (PHP and IOP) contracted by the County of San Diego, and substance use treatment certified by DHCS. We do not operate residential or inpatient beds; partner housing and coordinated referrals cover what we do not deliver on site, and we say so plainly when an assessment points that way.
