Acceptance and commitment therapy shifts the goal from eliminating difficult thoughts to acting on your values while those thoughts are present. It starts from an uncomfortable observation: the effort to control unwanted internal experience is frequently what keeps people stuck.

Avoid every situation that provokes anxiety and the anxiety wins by default. Drink to make a feeling go away and the feeling returns with interest. ACT redirects the energy from fighting the internal experience to building the life.

What the work involves

Defusion

Learning to see a thought as a thought rather than as a fact — noticing "I am having the thought that I will fail" instead of simply believing it.

Acceptance

Allowing a feeling to be present without spending everything you have on suppressing it. Not resignation, and not approval — just an end to the struggle that was consuming the resources.

Values

Getting specific about what actually matters to you, as distinct from what you think should matter or what would impress someone. This is the part people often find unexpectedly difficult.

Committed action

Doing the thing that moves toward the value, with the anxiety or the grief still in the car. Not waiting until you feel ready, because that wait can be indefinite.

What we use it for

  • Anxiety, particularly where avoidance has narrowed someone's life
  • Chronic depression that has not responded to symptom-elimination approaches
  • Substance use, where using functions as escape from an internal state
  • Trauma, alongside trauma-focused work

Where this fits in our programs

Therapy at Golden Coast Rehab is delivered inside a program rather than as a standalone service. That means our specialty mental health partial hospitalization and intensive outpatient programs, provided under our County of San Diego contract, and our substance use intensive outpatient and outpatient programs under DHCS certification 370254AP.

Which modalities you work with is decided with your clinician based on what you are dealing with, not assigned by a template.

FAQs About Acceptance and Commitment Therapy (ACT)

Does acceptance mean giving up on feeling better?
No. People generally do feel better in ACT — it just is not the target being aimed at directly. The paradox is that dropping the fight with the feeling usually reduces it more than fighting did.
How is ACT different from CBT?
CBT tends to examine whether a thought is accurate and change it. ACT changes your relationship to the thought instead, without needing to argue with its content. Both are useful and we use both.
Is this the same as mindfulness?
Mindfulness is one component. ACT adds values clarification and committed action, which is what keeps it from becoming a purely internal practice.
Who does ACT suit best?
Often people who have done CBT and found that disputing thoughts did not stick, and people whose main problem is avoidance rather than the intensity of the feeling itself.
