Psychodynamic therapy looks at how earlier relationships and unexamined patterns shape present behavior. Where CBT asks what you are thinking right now, psychodynamic work asks why this particular situation reliably produces this particular reaction in you and not in the person next to you.

It is less structured than skills-based approaches and it moves at a different pace. For some people it is the piece that explains why the skills kept not sticking.

What the work involves

Sessions are more open than in CBT. You talk, your therapist listens for the pattern rather than only the content, and over time the recurring shape becomes visible — the same dynamic showing up with a partner, a boss, and a parent.

The therapeutic relationship itself becomes useful material. How you relate to your therapist tends to reproduce, in miniature, how you relate elsewhere, and that is observable in the room in a way that a description of last week's argument is not.

In an intensive outpatient setting, psychodynamic work runs alongside more structured treatment rather than replacing it. If you are in acute crisis, stabilization and skills come first — insight is worth a great deal, and it is not what gets you through the next 48 hours.

FAQs About Psychodynamic Therapy

Is this the same as psychoanalysis?
Related but not the same. Psychoanalysis is far more intensive and longer-term. Psychodynamic therapy takes the core ideas and applies them in ordinary weekly-or-more sessions.
Do we just talk about my childhood?
Only where it is relevant to something happening now. The point is the pattern that is still operating, not an archaeology project.
How is this different from CBT?
CBT targets current thoughts and behaviors with structure and homework. Psychodynamic therapy works on the underlying patterns and the relationships that formed them, with less structure. Neither is better in the abstract; they suit different problems and different people.
Is it available in your programs?
