Narrative therapy separates the person from the problem, then helps them re-author the story they carry about themselves. Its central move is deceptively simple: the problem is the problem, and the person is not the problem.

The difference between "I am an anxious person" and "anxiety has been telling me to avoid things" is not word games. The first is an identity with nothing to be done about it. The second is a relationship, and relationships can change.

How the work goes

Externalizing

The problem gets named and treated as something separate from you, with its own tactics and its own preferred conditions. What does depression want you to do today? What does it tell you about yourself when you are tired?

Finding the exceptions

The dominant story is usually total — it always happens, you never manage. Narrative work hunts for the moments that contradict it, which almost always exist and have gone uncounted because they did not fit.

Thickening the alternative

Those exceptions get built into a fuller account: not a positive spin, but a more complete story that includes the parts the problem edited out.

Why it matters in this setting

Shame does enormous damage in both mental health and addiction, and the story people carry — that they are weak, that they are a burden, that this is simply who they are — is frequently the thing keeping them from treatment at all. Narrative work goes directly at that.

It also fits well with group therapy, where the alternative story gets witnessed by other people rather than only asserted privately.

FAQs About Narrative Therapy

Is narrative therapy just reframing things positively?
No. It does not ask you to describe hardship as good. It asks whether the story you have been told about yourself is the whole account, and usually it is not.
Who does this work well for?
People carrying heavy shame or a fixed identity story — particularly around addiction, where the label can become the whole self-concept.
Is there evidence for it?
It has a smaller evidence base than CBT or DBT and is used here as one approach among several, chosen where it fits, rather than as a primary treatment for acute symptoms.
Can it be combined with other therapies?
Yes, and that is how we use it — alongside structured, skills-based, and trauma-focused work rather than instead of it.
