Mindfulness-based stress reduction builds a practical, trainable capacity to stay with discomfort without reacting to it. It is a skill, developed through repetition, and it has nothing to do with belief.

The relevant gap for almost everyone in treatment is the one between a feeling arriving and a behavior following it. Craving to using. Anger to the sentence you cannot take back. Panic to leaving the room. Mindfulness practice widens that gap, and a wider gap is where every other skill gets a chance to operate.

What the practice involves

Attention training, mostly unglamorous: following the breath, scanning the body, noticing where attention has gone and bringing it back. Noticing that it wandered is the exercise, not a failure of it — a point worth repeating to anyone who has concluded they are bad at this.

Alongside formal practice, informal practice matters: doing one ordinary thing at a time with your attention actually on it.

What we use it for

  • Anxiety — noticing the escalation early enough to do something else
  • Craving — observing a craving rise and fall without acting, which it does, reliably, if nothing feeds it
  • Depression — interrupting rumination before it takes the afternoon
  • Chronic stress and sleep difficulty
  • As the foundation of the mindfulness module in DBT

A note for trauma

Sustained attention to the body can be destabilizing for some people with PTSD, particularly early on. That is a known effect, not a personal failing, and practice is adapted accordingly — eyes open, shorter, externally focused. Tell your clinician if a practice makes things worse rather than better.

FAQs About Mindfulness-Based Stress Reduction (MBSR)

I cannot clear my mind. Does that mean I cannot do this?
Clearing the mind is not the goal and never was. Noticing that your attention wandered and returning it is the entire exercise. A wandering mind is the equipment working normally.
Is this religious?
No. The practices used here are secular attention training, presented as skills.
How long before it helps?
Usually weeks of regular short practice rather than one long session. Consistency matters far more than duration.
Can mindfulness make things worse?
For some people with trauma histories, yes, particularly body-focused practices early in treatment. It is adaptable, and you should tell your clinician if it is having that effect.
